When is surgery used for glaucoma treatment?

In general, surgery isn’t the first line of treatment for glaucoma. But if other treatments fail, glaucoma surgery may be able to save your vision.

The goal of glaucoma treatment is to lower your intraocular pressure by helping fluid to drain. When the pressure is elevated, it can push on your optic nerve – leading to vision damage. A variety of medicated eye drops can be prescribed to reduce the pressure levels, but they aren’t effective for everyone, especially when pressure levels are very high and need to be lowered quickly or sometimes medicine causes serious side effects, such as hypertension, which makes it a poor option for glaucoma treatment.

In certain cases, Dr. Tu may recommend glaucoma surgery at our surgical center.

Minimally Invasive Glaucoma Surgery (MIGS)


Through small incisions or insertion of miniature implants, MIGS has become an ideal surgical procedure for glaucoma patients.

What is MIGS?

There is a framework of tissue near the cornea called the trabecular meshwork and is responsible for draining the aqueous humor from the eye. When the flow of fluids are clogged up, the eye pressure increases and can be the cause behind glaucoma. Through MIGS, an eye surgeon can allow fluids to drain properly from the eye & reduce the eye pressure.

If Dr. Tu thinks MIGS is right for you, the following procedures may be done:

  • Microtrabeculectomy
  • Internal trabecular bypass procedures
  • Selective laser trabeculoplasty

MIGS has become a preferred surgical procedure for glaucoma for the lower risk of complications and higher success rates compared to other glaucoma surgeries.

Conventional non-laser glaucoma surgery:


For more advanced glaucoma or glaucoma that is out of control, more invasive surgeries such as a trabeculectomy may be performed.

All of these procedures can often be perform in the comfort and convenience of our in-office surgical center.
